Clarity Hearing Care is committed to protecting the privacy and security of your health information. As we provide hearing care, we create and maintain records that may identify you and describe your care, services, insurance, or payment history.

This Notice applies to Clarity Hearing Care and all three of our clinic locations in Osprey, Englewood, and St. Petersburg, Florida.

You may request a list of certain disclosures of your health information made during the six years before your request.

The list will not include disclosures for treatment, payment, health care operations, or certain other exceptions permitted by law.

One accounting during any 12-month period is provided at no charge. A reasonable, cost-based fee may apply to additional requests.

◆ Choose Someone to Act for You

If someone has legal authority to act as your personal representative—such as through a medical power of attorney or legal guardianship—that person may exercise your rights and make choices about your health information.

We will verify that authority before taking action.

◆ Receive a Copy of This Notice

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

◆ File a Privacy Complaint

You may file a complaint with Clarity Hearing Care if you believe your privacy rights have been violated.

You may also submit a complaint to the U.S. Department of Health and Human Services Office for Civil Rights.

We will not retaliate against you for filing a complaint.

For certain health information, you may tell us your preferences regarding what we share.

We may use and share your information with audiologists, hearing instrument specialists, physicians, technicians, staff members, and other health care professionals involved in your care.

This may include coordinating care, making referrals, obtaining medical clearance, placing appropriate orders, or consulting with another provider.

◆ Obtain Payment

We may use and share your information to bill and receive payment from health plans or other responsible parties.

For example, we may provide information to an insurance company to verify benefits, obtain authorization, submit a claim, or determine coverage.

◆ Operate Our Practice

We may use and share information to operate Clarity Hearing Care, improve services, train staff, evaluate performance, manage business activities, and contact you about your care.

◆ Business Associates

We may share information with companies that perform services for us, such as billing, technology, document storage, communications, or practice-management support.

These business associates are required to protect your information appropriately.

◆ Public Health and Safety

We may disclose information for public-health and safety activities, including:

• Preventing or controlling disease or injury.

◆ Products on Display or Available to Order

Clarity Hearing Care maintains demonstration or display models of many hearing aids and accessories shown in our online catalog.

Products may be available for patients to see, examine, or experience during an in-office visit at our Osprey, Englewood, or St. Petersburg location.

Display and demonstration units are not normally sold as the patient’s final product.

After a patient selects an appropriate hearing aid or accessory, Clarity Hearing Care may order a new unit from the manufacturer for delivery to the selected office.

We will contact the patient when the product is ready and arrange professional fitting or local pickup, as applicable.

📍 Important Availability Information

A product identified online as “on display” or “available to order” may not be available for immediate take-home purchase.

Display models, brands, accessories, styles, colors, and sizes may vary among the Osprey, Englewood, and St. Petersburg offices.

Please call your preferred location before traveling if you would like to see a particular product.

◆ In-Stock Domes and Ear-Tip Supplies

Common hearing-aid domes, ear tips, and related supplies may be kept in stock at our Osprey, Englewood, and St. Petersburg offices.

Available manufacturers, styles, and sizes may vary by office and current inventory.

Please call before visiting if you need a particular manufacturer, style, or size.

◆ Estimated Delivery Times

• Standard hearing aids and accessories: approximately 3–5 business days.
• Custom hearing aids, custom earpieces, and earmolds: approximately 7–10 business days.

Manufacturer backorders, holidays, severe weather, customization requirements, shipping delays, or other circumstances outside our control may extend these estimates.

We will explain any known delay before the patient approves the order.

◆ Delivery and Shipping Costs

Standard manufacturer delivery to a Clarity Hearing Care office for professional fitting or local pickup is provided at no delivery charge to the patient.

If expedited delivery, direct accessory shipping, or another special shipping method is requested, any additional charge will be disclosed before the order is placed.

Product prices and any applicable professional-service, fitting, customization, or special-order charges will also be explained before the purchase is approved.

◆ Professional Fitting and Local Pickup

When an ordered product arrives, Clarity Hearing Care will contact the patient to arrange an appointment or pickup time.

Hearing aids and prescription hearing devices are fitted and picked up through a Clarity Hearing Care office and are not ordinarily shipped directly to patients.

Eligible nonprescription accessories and supplies may be available for local pickup or shipping when offered.

Any applicable shipping charge and estimated delivery time will be provided before the accessory order is completed.

◆ Availability Confirmation

Product availability can change between website updates.

Please contact Clarity Hearing Care to confirm that a particular demonstration model, hearing-aid accessory, or in-stock supply is available at your preferred office before visiting.

RETURNS, PAYMENTS AND GENERAL POLICIES

🔁 Return & Exchange Policy

We offer a 30-day full money-back return privilege on all hearing aid purchases.

  • Return period begins the day of in-person fitting

  • Returns are accepted for any reason

  • 20% restocking fee on all returned chargers.  All boxes, and other materials need to be in excellent condition in order for us to accept a charger for return.  Please make sure your ordering the correct charger.

  • Exchanges for other models are welcome

  • Custom earmolds are non-refundable unless defective

Our goal is to make sure you're satisfied. If you're not, we'll adjust, exchange, or refund — no hassle.

We accept the following payment methods in person:

  • CareCredit Financing (subject to approval)

5. Warranties

  • Manufacturer warranties apply to most hearing aids and accessories.

  • We do not provide any additional warranties beyond those offered by the manufacturer.

6. Appointment Policies

  • Appointments booked via our website (using CounselEAR) are subject to confirmation.

  • Missed appointments without 24-hour notice may incur a fee.

  • All medical evaluations and device fittings require in-person visits.
